Our Supported Housing with Care and Support provides individuals with a flat or house share complemented by tailored support. The level and type of care/support are personalized to your specific needs. Our service employs a person-centred care approach, aiming to assist individuals in maintaining their tenancy, identifying strengths and goals through the recovery model, enhancing self-esteem, self-worth, and confidence, and facilitating integration into their local community.
We offer a variety of properties, ranging from shared homes to self-contained flats, most of which come furnished with essential amenities.
Working collaboratively, our goal is to support you for a two-year period, aiming for a smooth transition to our medium support accommodation or referral back to mainstream accommodation with floating support/long-term low support services. Onsite staff are available 24 hours a day.
Our medium support serves as a transitional stage between 24-hour support and mainstream or low support accommodation.
With onsite staff available from 9 am to 9 pm, we guide individuals on their recovery journey.
This includes support for mental health, learning disabilities, physical health, autism, homelessness, substance recovery, criminal justice system involvement, and young people with care needs.
Our Floating Support services empower individuals to live independently or maintain their existing living arrangements.
Specialised recovery focus workers provide assistance with risk management, monitor medication adherence, oversee well-being, facilitate and support appointments, promote social inclusion, and maintain tenancy. In times of crisis, individuals have access to a 24-hour on-call service.
Hours are allocated based on assessment outcomes, with additional crisis support available when needed.
The Crisis House offers short-term support for individuals facing acute mental health crises.
Accessible to adults aged 18 and above, our 6-bed facility features communal areas like a kitchen, lounge, and garden, with 24/7 staffing. Emotional support, guidance, and the development or re-engagement with coping strategies and daily activities are key components of our person-centred approach, ultimately focused on improving safety and well-being.
